As a broker/introducer, we recommend that you use our standard Credit Application Form, incorporating a 'Loan Purpose Declaration' in section G of it However, you are not obliged to use the standard form You can utilise your own form as long as it includes at least the information requested in the Quantum Credit Form.
In addition, a 100 Point Identification Form and a 'Loan Purpose Declaration' must accompany, or form part of, any application from a broker.
